For students who are taking the College Level Examination Program (CLEP), after completing and signing the form, they have to fax the form along with the required means of identification to the 

College Board’s Services for College Level Examination Program (CLEP) at 610‐628‐3726. Or, the completed form alongside the required means of identification can be emailed as an attachment to clep@info.collegeboard.org.
